Orlistat (orlistat) is a lipase inhibitor used to help patients lose weight or keep lost weight off. It accomplishes this by interfering with fat absorption during digestion. Like most weight loss medications, results are enhanced by good diet and exercise.

Losing Weight with Orlistat

Orlistat comes in capsules of two strengths: 60 mg and 120 mg. 60 mg capsules are widely available over the counter; however, many areas require a prescription to buy orlistat in 120 mg strength.

Regardless of strength, treatment is the same: one capsule is taken with a meal, if the meal contains fat, 3 times daily. It may also be taken up to 1 hour after a meal. 

In 60 mg doses, about 25% of the fat be eliminated undigested; around 30% is undigested with 120 mg doses. There is no additional benefit shown in taking doses higher than 120 mg. There is no benefit to taking orlistat with a meal that does not contain fat.

Treatment may last several months, though rarely over a year continuously. Long-term use may interfere with absorption of certain vitamins and nutrients, which may need to be supplemented with a daily multivitamin, which should be taken at least 2 hours before or after taking orlistat.

Results with this medication are modest unless used in conjunction with a reduced calorie diet. Fats, protein and carbohydrates should be evenly distributed over the three primary meals of the day.

Precautions & Side Effects

Though widely regarded as safe, there are a few situations in which orlistat should not be used:

• In patients with kidney or gallbladder issues
• In Patients with underactive thyroid issues
• In patients with eating disorders such as anorexia
• In patients who have difficulty absorbing vitamins or are malnourished

Orlistat may be harmful to unborn and nursing babies, as it will interfere with vitamin absorption. Women who are trying to become pregnant should also avoid use; women who may become pregnant should use a contraceptive for the duration of treatment with orlistat.

Side effects with orlistat are rarely dangerous, though a few are unpleasant. The medication's most common, and notorious, side effects are gas which may leave oily spotting on clothing, and difficulty controlling bowels. The odds of experiencing either of these side effects are dramatically reduced by reducing the amount of fat eaten in a meal; fat intake should be distributed evenly between three meals and never exceed 30% of any one meal.

Less common side effects include:

• Stomach upset
• Diarrhea
• Headache
• oily stools
• Frequent bowel movements
• Light colored stools
• Changes in menstruation
• Fatigue

Use of other medications with orlistat is unlikely to cause dangerous interactions, but orlistat may interfere with their absorption. Check with a doctor or pharmacist, or, if unsure, take other medications at least 4 hours before or after taking orlistat.
